Once I had downloaded it, he had me open it, and click Network> Join an existing network. This is what you would do if someone else has a server open already that you want to join. When you do this, you put in the name of the server, and the password for it, and that will get you onto the server, and will give you the IP addresses of those on the server who have Minecraft worlds open on that server. You put that IP address in under Multiplayer, and if they have their multiplayer world open, you'll go to it.
If you're creating your own new server, I'm not 100% sure what you'd need to do, but my guess, is that you'd simply go to Network> Create new server, then set up a name and password for your server, and have friends do the above steps. There may be more to it though. Either way though, you're going to have to know who it is you're going online with, and have contact outside of Minecraft itself, if you want to play online with people. Everyone needs to know the person hosting the server in order to play on their world, but not everyone playing in that person's world needs to know each other. Good luck!

A friend of mine helped me to do this the other day, actually. I'm sure there are multiple ways to do it, but this was the way I was shown.

find a server that isn't white listed, find the ip address. and go into minecraft. and type the IP address into the multiplayer thing.
Try looking for a server. You can find some servers to join at minecraftforum.net.
