How do you write a professional bio for a musician?

Do you have a program from a concert? You can look in there to see how others wrote theirs. You don't need to put your address, phone, and age in it - I've never seen a performer's addy, phone, or age in a concert program.
I would just give your name - then make sure the paragraph hits these pionts:
possibly when you started performing and what brought you to choose your instrument,
where you studied,
each degree you received and where,
the schools you went to,
any specialized training you received (when and where and with whom),
any studies abroad (when and where and with whom),
whom you studied under,
how long you've been performing,
where you've performed before and with whom (if you've played in a band or orchestra or chorus, name them and the city),
if you're currently studying with someone, put their name down
Then end the paragraph by stating any personal info you want to share - if you're married, if you have any kids, where you live (though not necessarily your addy) - i.e. Jane Doe lives in Jonesville with her husband of 3 years, John and their 2-year-old son Jim. She plays with the Symphony Orchestra in addition to her work as a full time mother. John works as an engineer for ______ company.

EXPERIENCE (concerts,perhaps-not sure-teaching exp.,participation in contest-if you've ever been awarded...)
You write it in sections e.g.PERSONAL INFO
