Contact your local active recruiter to get the DD368 initiated and take it to your readiness NCO along with your memo. Ask your readiness NCO for the company letterhead to make it look more official. Keep it short somewhere around a half page. Good luck.

You can certainly include that on the form but whether or not you get a transfer is up to the needs of your unit, not your financial issues.
