Here in the states even in the very high crime cities, vest are not generally issued. Providers who wear vests generally own them and choose to wear them.
Since part of their job consists of lifting patients onto a gurney, and sometimes people are very heavy..or they have to lifted on a board to keep their body/head straight..they likely wear a back brace of some kind (like the grocery store workers and UPS guys wear) would be my guess.

Since they show up with the Fire Department, it's possible they wear protective vests under there, as Law Enforcement and Firefighters wear.
