Who do I report my employer too?

I have been paid on time maybe twice in 8 months. Our last pay check was five days late and then bounced when we tried to deposit them. We have also not received our w2's and cannot get a straight answer from our employer as to where they are. I needed to do my taxes for the financial aid forms that were due days ago. Which means I am screwed for going back to college next year. Who do I report them to? Also, I live in Colorado if that makes a difference.
