PTSD is a mental issue. It occurs after a traumatic event, causing the victim to be constantly anxious and sometimes feel the danger is still prevalent. While this is a mental issue, it could effect the person's body. For example, some one with PTSD could end up using self harm or an eating disorder to cope, which in turn effects the body. So generally mind, but could be body too.
PTSD is a psychiatric disorder - this means that while it is a disorder of the mind, there are underlying chemical imbalances which form a basis for the disorder hence, it, like all psychiatric disorders it has a physical root. Because PTSD stems from trauma it also has a strong mental component. Therefore, PTSD is of both body and mind. It is a mental health injury that has symptoms that can manifest themselves both mentally and physically (trembling, jumpy, palpitations etc)

It is a mental issue. But like many mental issues, there is a real physical change in the brain chemistry.
