Do dermatologist really hire paramedical estheticians?

I'm thinking about going into the esthetician field. I don't want to work in a spa or in a medical spa. I'd rather work in an a dermatologist office. That seem more like a "real job" to me. But my questions I want to know is, how impossible is it to find a job work with a dermatologist? Are they really necessary?
