After you beat the elite four, head from Nimbasa City to White Forest/Black City (depends on version) On the way there, there will be a building. Inside the building is a think called Poke-Transfer. It will walk you through the steps in order to transfer your pokemon. You do need two DS's, however.
Yes, with the transfer you chose the pokemon you want from the old game, and play a minigame to catch the pokemon you chose to transfer, you must catch the pokemon to get them in the minigame, then they get transfered to black or white, you can do this as many times as you want, however, once you transfer you cannot transfer them back to the old game you transfered from
