Dosage Calc Test....................?
Taking the dosage calc test tonight and know it all except for the very last question on the practice test. If anyone could explain it I would appreciate it!

Directions: Reconstitute Zosyn with 5 mL of normal saline for a total solution volume of 5 mL.

ok, so when reconstituted, you will have 3.375g in 5ml
3.375/5ml = 2.5/x
2.5 times 5 = 3.375x
12.5 = 3.375x
x=3.7
so, the answer is 3.7 ml will be 2.5g
3.375 gm / 5 ml = concentration
0.675 gm / ml = concentration (divided the figures in the line above, same as 0.675 gm / 1 ml )
Dose to be given = 2.5 gm
2.5 gm / 0.675 gm/ml = dose to be given in ml
3.7 ml = Answer
Careful people with a conscience who are doing medical work always CHECK . They check the source bottle to make absolute, as in ABSOLUTE, sure they have the correct item. The KNOW some (hopefully a lot) about the stuff they are giving, so that there is No Mistake, because sometimes there is mistake made in writing by busy doctor, computer error, or verbal miscommunication.
If you do not know what is bad, you are a GREAT DANGER. For babies, even smaller doses are involved.
Those who are weak and unsure at MATH are the GREATEST DANGER OF ALL, unless they check very carefully to be ABSOLUTE SURE. When there is ANY DOUBT, you should seek help from someone with greater drug AND MATH competence.
So this must be CHECKED now as follows:
3.7 ml of 0.675 gm per ml to be given
3.7 (0.675) = 2.5 gm
Check is okay. Now you may administer the medicine.
If you do not do this, very carefully, just as YOU would like your caretaker to do for you, then you are a criminal and lazy or both.
BTW, people who do not get this message make mistakes and kill people, and yes, sometimes even babies.
Sometimes they are not killed but have a very adverse reaction with recovery needed and possible permanent injury.
I did LOTS of medical work and saw this again and again, even in senior nurses sometimes.
Even worse is messing up similar names, like formaldehyde and paraldehyde . A fool who is also stupid should know that formaldehyde is not a medicine. But believe it or not, even that has been brought to bedside and administered to convulsing patient.
BY THE DOCTOR, who just grabbed the bottle from the ignorant person who (mis-) fetched it, and administered without reading the label.
That is WHY you ALWAYS read the LABEL carefully twice.
Plus in this example there are two stupid working who are so dumb they should be banned for life from any medical work.
The nurse who is most competent at math should be the one who checks things that must be calculated, even if someone else is giving meds. And if someone is weak, they are WISE by getting other person to check their result. Then you have a WITNESS that it was done right.
People who do their work with QUALITY never have bad conscience nor trouble sleeping.
Even the higher judges will notice this...
Now, after all of that, I can tell you from experience that RIGHT NOW there are some working who are not careful, too hurried, in doubt, did not read label twice, and are weak at math, and do make mistakes. They are working right now and viewed as trusted professionals.
When it becomes just a little more complicated than usual, that is when the mistake is more likely. Voice of long experience.
And when they are NOT SURE of themselves, but go ahead anyway. And being 'in a hurry' is NO EXCUSE for not checking in the always prescribed manner universally taught.
