Look online for free or low income clinics but a sports physical is just a basic physical. They check you eyes, ears, nose, mouth, blood pressure, weight, hearing etc. Ask your coach what exactly the physical needs to be tested for but you can try to find cheap ones at your community clinic or even planned parenthood.
It is however much the doctor you picks decides that to charge. If you decide to use a doctor who wants to do it for free, then it is. If you decide to use a doctor that wants to charge $10 million dollars, then it is $10 million dollars. There is no minimum or maximum. It is up to each doctor how much that doctor charges, and it is up to you to decide how expensive a doctor you want to use.
