Exactly what the question says.. does it come with Vista Premium? or do I have to custom make a computer with Media Center on there?

Do you mean, "Does Vista Home Premium come with Media Center?"? Yes it does. Media Center comes as a component of Microsoft Windows Vista Home Premium and Ultimate versions (both 32bit and 64 bit). Home Basic and Business do not have Media Center.
