Depends on the vehicle but generally speaking you need a manual transmission, clutch assembly, and a clutch pedal. You may also need a clutch master cylinder if this system is going to be hydraulic, new driveshaft/side shafts and a new engine control module, again,depending on the vehicle.

depends on the car, and the gearbox
but, you could simply swap the auto for a manual 'box
or you could do a manual conversion, e.g. like on some drag cars
keep in mind this is not a straightforward thing to do, have to consider the car's engine, driveline etc.
