Does preventing suicide sometimes fail?
A friend of mine was driven to suicide, but others prevented it much to his chagrin. Though they saved my friend he didn't want to be saved. His family and friends tried getting him help only for him to refuse it. Basically, my friend had grown sick of life and the struggle. He was burning with hatred. He refused to work. He refused to be part of society. He refused company. In the end, he ended up living on the streets, got into drugs, and he's now in prison, which I learned actually pleases him. He's unwilling to live, so he figures it's better to die or rot in a cell. Honestly, I agree with him. Life isn't for everyone and some shouldn't be forced to live it. People tried to help him, he didn't want it, so they failed.

Humankind is wired to fight to live. That's how we, the species, survive. It is abnormal to want to die. If it were normal then the species would have died out long ago.
But I agree. We can only offer so much therapy; otherwise it becomes prohibitively time and money wasting. So after giving it a valiant try for salvation, it's time to let your friend do his own thing. Time to move on.
Yes,it does since a person can't be watched 24/7 unless they are committed.
