ok we are 50% sure he's dead , because Edo Tensai was used on him , however Zetsu ( who can make clones ) and Muu ( who can split ) , might have had something to do with his resurrection , i am pretty cure he's dead and there is someone else behind the mask
as for your second question this is what all the naruto readers are going frantic about , possible theories are Madara himself ( i personally disagree with this ) , Izuna ( madara's younger brother ) , Rikudo Sennin older son , Hashirama , Tobirama ( unlikely ) , Madara's son and obito ( i believe this wrong obito's body may have been used but there is no way he's the guy behind the scenes , he was just a little boy who taught kakkashi how to live ), Sasuke and itachi's father , Kagami, shisui , Jubi , well you get my point here everyone is going crazy thinking about this , we still don't know who Tobi is and these theories will keep up until we do .
1. Well since he was revived by Kabuto, its very apparent that he was dead.
2. I don't know who is behind the mask. I know it can't be Obito. Tobi would be to old and if they made it Obito, they would have to explain a lot because that would be a big story gap.
I'm very annoyed with the latest development. Not because it was bad but because we now have to wait to find out who the heck tobi is, and I hate waiting. Patience is not my virtue.
2. Personally I believe it's Obito, Kakkashi's old friend who died. I'm guessing Zetsu could have used his remains too make a clone with that white stuff on his body and such. But hey, it will all be revealed next chapter possibly. I'm so excited O_O!!!
